Six – 1-hour Coaching Sessions

Service Description

This package includes 6 sessions. Each session is 60 minutes.  1st Session: Intake Session, discussion of modifiable customized plan and homework assignment to be completed before next session.  2nd through 5th Session: follow up session that will build from the previous sessions while setting the tone for the final session.  6th Session: Final session, discussion of goals reached, and unreached, new goals that have developed, and maintenance plan.
